To get a Level 4 Limit Break you must first gain all the characters other Limit Breaks, up to Level 3 and then find a Manual to activate the Level 4 Limit Break.

Cloud Omnislash Purchase at Battle Square (Gold Saucer)
Tifa Final Heaven On either Disc 2 or 3 play the song in Tifa's House (Nibelheim)
Barret Catastrophe After saving North Corel you gain Catastrophe from an old lady in a hut (North Corel)
Aeris Great Gospel Speak with the sleeping man near Junon, if the amount of battles you have fought total to a number where the last two digits are the same he will give you a piece of Mythril, you can then exchange this for 'Great Gospel' with the Weapons Seller near Kalm.
Note: This must be done before the end of Disk One!
Red XIII Cosmo Memory Defeat Lost Number Boss in ShinRa Mansion (Nebilheim)
Cid Highwind Find it along side the Hades materia and Double-Cut materia in the Sunken Airplane, which can be found be going underwater in your submarine near the Gold Saucer.
Yuffie All Creation Defeat Godo (Wutai Pogoda)
Vincent Chaos After getting the submarine or Gold Chocobo go to Lucrecia's Cave, it is located behind a Waterfall in the Nibel region.
